Meet with WWM Team for Initial Health Behavior Assessment, Measurements, EKG (if needed) & review JumpStart diet week 1-2, Optional Genetic Testing + cost of food/injections.

WWM education visit. + cost of food/injections
Phase 2
Level I Visit
$30 Education with Wellness Coach
Level II Visit
$60 Education with Wellness Coach and physician evaluation as indicated for diet modification and initiation of medication management
Weeks 5-6
WWM education/BP/Script f/u + cost of food and injections
